Initial commit - MSMD Player with bug fixes

Bug fixes applied:
- Fixed AttributeError when showReferenceCreator is disabled in config
- Fixed keyboard input with modifiers (Shift+key) not being recognized
  by making scancode table lookup case-insensitive
